Dr. KholokiAt Plymouth Place, caring for residents has always meant more than responding to medical needs. It means knowing their stories, nurturing their independence, and supporting a life filled with art, purpose, joy, and connection. Now, with the launch of Plymouth Concierge Care, Plymouth Place is taking wellness to a whole new level. Key to the whole program is longtime partner and Medical Director Samer Kholoki, MD.

“With Dr. Kholoki at the helm of this innovative program, we truly are raising the bar on health and wellness at Plymouth Place,” said CEO Jay Biere. “This isn’t just about treating illness. It’s about elevating day-to-day living, supporting independence, and giving residents and their families real peace of mind.”

Having worked with Plymouth Place for more than three decades, Dr. Kholoki agrees. “People don’t come here for the chandeliers,” he said with a smile. “They come for the quality of life – and the exceptional care that helps them stay active, confident, and engaged in the community they love.”

Practicing Medicine the Way It Was Meant to Be
For over 35 years, Dr. Kholoki has approached medicine with a blend of compassion, clinical excellence, and rigorous planning. “Serving our residents requires knowing their story,” he said. “When an urgent visit arises, meaningful recommendations come from preparation. An accurate medical file leads to effective decision-making and safe outcomes.”

With the Plymouth Concierge Care program, every resident at Plymouth Place will have a comprehensive, up-to-date medical summary on file — including past medical history, current conditions, and a full medication list. They must also complete an annual wellness exam with Dr. Kholoki. What’s more, this program goes beyond medicine, including annual in-home safety evaluations to alleviate fall risks and assure each resident’s personal comfort and security.

This program does not replace existing physicians, unless a resident chooses that path. Instead, it ensures continuity of care and that a qualified medical professional is always ready to step in if a resident has an immediate medical need and their physician is unavailable. With unique expertise caring for patients in assisted living and memory care, Dr. Kholoki and his team will be on campus five days a week, and eventually, two Saturdays a month.

What’s more, there is no additional charge for this service. Annual wellness visits, labs, and related tests are fully covered by Medicare. If hospitalization is ever needed, Dr. Kholoki personally oversees admissions at La Grange Hospital and Hinsdale Hospital. “We refuse to turn our patients over to hospitalists who don’t know them,” he said. “Continuity and personalization matter.”

Unlike typical medical programs today, Plymouth Concierge Care introduces measurable benchmarks around overall health, mental function, cognitive skills, and safe mobility — with families actively involved every step of the way. “Together, we track progress, celebrate improvement, and adjust the plan,” said CEO Jay Biere. “It’s a level of partnership that truly differentiates Plymouth Place.”

For Dr. Kholoki, measurement is essential. “It’s not enough to say, ‘Mom is doing fine,’” he said. “We need numbers. We need to see if her joint health, mental health, and mobility are improving. That’s how we add life to years — not just years to life.”

To support continual improvement, residents receive personalized “homework” — from memory games to range-of-motion, and balance exercises. Even small commitments can be transformative. “One study showed that seven minutes of range-of-motion exercises a day can reduce fall-related ER visits by 80%,” Dr. Kholoki noted. “When we meet with families, they can compare the first quarter to today. We measure the gains — and they can see real progress.”

This quantifiable, results-driven model earned him recognition from the American College of Physicians for exceeding national benchmarks. Plymouth Place’s onsite physical therapy team amplifies these outcomes, helping residents restore strength, prevent falls, and maintain mobility. “Our residents deserve the best of clinical care and the best of community living,” Biere said. “This program brings those worlds together.”

Whole-Person Care: Body, Mind, and Spirit.
Unlike many providers, Dr. Kholoki takes a hands-on, holistic approach that goes far beyond medication management. His care is incredibly human, taking the time to listen and respond quickly for the most effective course of action.

“In England, where I trained, a GP is expected to be deeply involved,” he said. “If you wait four or six weeks to see a specialist, that could be detrimental. Acute care cannot wait.” Dr. Kholoki’s commitment to addressing issues as soon as possible is key to preventing long-term decline. It also builds strong relationships that redefine the patient care experience.

A trained cellist, Dr. Kholoki once helped a resident download Bach to her iPad and prescribed that she listen to the playlist every morning. “Her daughter told me, ‘You have no idea what this has meant to her,” he said. “’She’s in different spirits and more at peace when we talk to her.’”

He remembers another resident who was 71 on the day he met her. “She asked me how long she would live. I told her, ‘If you make it to 100, I’ll bring the chocolate cake myself.’ When she turned 100, her daughter called me and said, ‘I have a note here that my mom laminated saying you would buy her chocolate cake when she turned 100.’ I brought the cake, and we had a wonderful celebration for her at Drury Lane.”
