Welcome to Transitional Care at Plymouth Place, where you can enjoy exceptional, around-the-clock care, tailored to your needs, from our dedicated team of caregivers and support staff.

Transitional Care is a neighborhood of 13 well-appointed apartments on the northeast corner of Plymouth Place’s first floor. Each of these spacious, private studio residences has a bathroom equipped with a walk-in shower, and housekeeping, linen and towel service are included.

Our main caregivers—called resident assistants, or RAs—are here for you 24/7 with planned events and other scheduled and unscheduled activities to keep you engaged and involved in the community. The RA team is supported by our life-enrichment staff, volunteers, social services staff, and nurses. As part of our Assisted Living program, Transitional Care has licensed staff members ready to help with your medication-management needs.
